Active Workforce Lifestyle Referral Programme

The Lifestyle Referral Programme is aimed at improving the health and well-being of employees, with a particular emphasis on those who may suffer with health problems that affect both their working and daily lives. As part of Sefton’s nationally acclaimed ‘Active Workforce Programme’, the Lifestyle Referral Programme is available to any employee from an Active Workforce partner organisation. 

What is the Programme?

• A 14-week supported lifestyles programme under the guidance of a fully qualified Occupational Health Referral Officer.
• A specialised programme of physical activity tailored to improve specific health conditions and designed to meet individual needs and desired goals.
• Includes health and well-being screening before, during and after the programme to assess health benefits and general progress.
• Gives subsidised use of all Active Sefton Leisure Centres (*£1.50 per session).
• Offers the opportunity to take part in the programme alongside a buddy (partner, carer or friend) at the same subsidised price, to increase motivation and commitment.
• Leads to heavily reduced price membership as part of the Active Workforce Programme after completion.

 What are the Benefits?

  • Better control or reduced chance of developing high blood pressure
  • Helps control diabetes
  • Aids weight loss, improves body shape and increases muscle tone
  • Increases bone density, muscular strength and endurance
  • Improves respiratory function

  • Decreases anxiety, stress and depression
  • Improves self-esteem, confidence and independence
  • Helps improve sleep
  • Allows you to meet new friends
  • Improves mood and productivity
  • Improves both physical and mental wellbeing, reducing the chance of future poor health

 Who can access the Programme?

The Programme is available to any employee who suffers from health conditions listed, which can be improved through a healthy lifestyle and is employed by an Active Workforce partner organisation. 

Referrals can be done in two ways:

  • Through the Occupational Health Team
  • Through the Human Resources Team in partnership with Active Workforce

To access the programme you must exhibit one of the following:

  • High blood pressure
  • Angina (treated and stable)
  • Musculo-skeletal problems
  • Anxiety / stress / depression
  • Post heart attack or heart surgery
  • Diabetes Type 1 & 2
  • Impaired Glucose Tolerance
  • Overweight / obesity (BMI over 25)
  • Back problems
  • Bone and joint disorders

OR

Two of the following:

  • Smoker
  • Hyperlipdaemia
  • Family history of Heart Disease
  • Asthma (treated and stable)
  •
